Meet the Folts Homes Health Center Team!

At Folts Homes, we believe our greatest asset and the cornerstone to our success is our staff members.  Each employee in every department makes a difference in the quality of care provided to our residents.  The five skilled nursing units of Folts Health Center features neighborhood-style settings that are designed to give senior residents and staff members more opportunities to share their daily lives in a way that feels like home. 

The 40-bed units on the first and second floors are designated for short-term and/or long-term rehabilitation.  Inpatient short-term rehab is an intensive short-term rehabilitation for orthopedic, neurological and cardiac patients.  This program is for seniors with the potential to improve mobility and independence and return to their homes.  The long-term rehabilitation program focuses on assisting residents to maintain strength and improve functioning skills through physical, occupational and speech therapies.

The staff on the third, fourth and fifth floors are specifically trained to care for and meet the needs of those individuals with memory impairment.  Folts Homes has created a calm, safe and secure environment suited for our residents whose lives have been changed by Alzheimer’s disease and dementia. Family members can be assured that their loved one is being taken care of physically, mentally and spiritually by our professionally trained staff.

Every weekday morning, a multi-disciplinary team (titled the “Morning Report Team”) meets to communicate clinical concerns, changes, admissions/discharges, and any pertinent issues that may impact the residents living at Folts Homes.  Representatives on this team include the nurse practitioner, director of nursing, nurse managers from each unit, PT, OT, dietary, chaplain, administration, activities, social services, admission coordinator and professional services (Medicare and MDS Dept.)
