Folts Homes Adult Day Health Services

Adult Day Health Care provides a program of services for adults who do not require 24 hour care, yet are not capable of full-time, independent living. The center offers a multi-disciplinary program designed to assist the individual to obtain his/her maximum level of health, prevent further disabilities, and provide respite for family members/caregivers.

Our Adult Health Care Center is designed and developed to meet increasing health care needs. As experienced providers, Folts Homes' services offer a team of health care professionals who are capable of meeting the medical, nursing, therapeutic, and recreational needs of each day care participant. Our programs enables each participant to keep living independently and safely. This program offers family caregivers respite, the opportunity to keep working, and the support to maintain your family structure.

* Coordination of individualized plan of care with primary physician on-site visits at physician's discretion.
* Health monitoring by a registered nurse.
